Reorder fields and add a single text line.

I recently purchased the Upload Form and I was going to use it for a small company’s website. They would like to use the widget to have prospective employee upload resumes through the widgets. Is there a way that I can change the fields in the form? I would like to be able to reorder the fields and add a single line text box so that prospective employees may input the job that they are applying for.

I am afraid you cannot change the fields in the form or reorder them.
A suggestion for the single line text box could be to use the message box.
